سابقه و هدف: ژن NOD2/CARD15 واقع در ناحیه کروموزومی(IBD1)16q12 به عنوان اولین ژن همراه با بیماریهای التهابی روده، به خصوص بیماری کرون مطرح شده است. بسیاری مطالعات همراهی متفاوتی از جهشهای این ژن با بیماریهای التهابی روده را در جمعیتهای مختلف گزارش کرده اند. در این مطالعه، فراوانی سه جهش شایع ژن CARD15 را در بیماران ایرانی مبتلا به بیماریهای التهابی روده، در مقایسه با افراد شاهد بررسی نموده ایم.روش بررسی: در مطالعه ای مورد - شاهدی ،40 بیمار کرون، 100 بیمار کولیت اولسرو و 100 فرد سالم که از نظر سن و جنس با بیماران همسان بودند، طی یک سال (83-1382) در بیمارستان طالقانی مورد معاینه و نمونه گیری قرار گرفتند. بررسی سه جهش شایع ژن CARD15 (G908R، R702W و1007fsinsC ) بر روی نمونه های DNA به روش PCR (polymerase chain reaction) و RFLP (Restriction fragment length polymorphism)انجام گرفت.یافته ها: میانگین سنی در بیماران کولیت اولسرو 38.6±14.3، در بیماران مبتلا به کرون 36.6±14.1 و در گروه شاهد 38.6±14.2 سال بود. در بین سه جهش بررسی شده فقط فراوانی جهش R702W در بیماران ایرانی مبتلا به کرون در مقایسه با گروه شاهد، بیشتر بود (3/16% در برابر 0/1% ؛ P<0.001، OR=19.2؛ 95% CI=4.2-87.3). تفاوت فراوانی دو جهش دیگر در بیماران کرون نسبت به گروه شاهد از نظر آماری معنی دار نبود. هیچیک از این سه جهش در بیماران مبتلا به کولیت اولسرو بیش از افراد سالم نبود.نتیجه گیری: با توجه به نتایج این مطالعه، به نظر می رسد در بین سه جهش بررسی شده ژن CARD15 فقط فراوانی جهش R702W در بیماران ایرانی مبتلا به کرون در مقایسه با گروه شاهد، بیشتر بود.
